What appointments can you take me too?

Community Health Nurses can provide escorts to:

  • Hospital Discharge follow-up
  • Initial Doctor Visits
  • Cancer Visits
  • Surgical Visits
  • Infants for all medical appointments
  • High Risk Children
  • High Risk Pregnancy

Community Health Representatives can provide escorts to:

  • F/u Doctor – Patients needing advocacy
  • Patients with Physical/Mental Limitations
  • Cancer Patient who Require Chemotherapy or Radiation
  • Wound Clinic
  • PT/INR Depending on Ranges – CHR for First Visit then referred to Transportation
How much time do I need to call in before I can ask for a nurse to take me to an appointment?

24-hour notice is needed

Which appointments require transportation?

Transportation/Taxi/Family Member is needed for these appointments:

  • Laboratory
  • Mammograms
  • Radiology
  • Vision Screens
  • Dental
  • Podiatry
  • Hanger
  • Out-Patient Surgery
Will you pick up my medications?

Yes, please call 480-768-2033 to request a medication pick up. The Nursing Program will be able to pick up and deliver medications to you within 48 hours.
